One of the more common conditions I see in the clinic that causes a lot of concern is "pink eye" otherwise known as conjunctivitis. Conjunctivitis just means "inflammation of the conjunctiva".
The conjunctiva is the membrane that lines the inner eyelid and the surface of much of the globe and is generally transparent. When it is inflamed it looks pink or red, hence "pink eye". While conjunctivitis is characterized by a red eye not all red eyes are conjunctivitis. Common causes of conjunctivitis include infection from bacteria or viruses. Allergies are also a common cause. Bacterial conjunctivitis is spread by direct contact. Patients typically complain of redness and discharge in one eye. The discharge is typically thick. Viruses also cause similar discharge but it tends to be more watery. Allergies present with similar symptoms as viral causes but sometimes can be characterized by the degree of itchiness. Other more rare causes include dry eye, chemical irritation, or foreign bodies. Red flags symptoms that should be addressed immediately by an eye medical doctor otherwise known as an ophthalmologist include:
In conclusion, if you find that you have signs or symptoms of "pink eye" see your doctor for further evaluation but don't assume that you need an antibiotic or that bacteria are to blame. Of course, each individual case is different and the advice in this post should not substitute for getting a consultation with your doctor. Over the past 50 years we have seen remarkable advances in technology. Has your doctor kept pace?
The answer is most definitely not. In fact, the level of access to doctors has probably decreased over the past 30 years. Most doctors will not engage with you over email, text, or phone unless they have too. One of the reasons for this is that insurance companies don't reimburse doctors for these types of communication. Most insurance requires in-person visits so that insurance can be verified. But there is a better way. I have helped a couple patients over the past week that had acute non-emergent conditions. One patient sent me a text picture of their throat. Another we communicated over text and phone. Both of these issues were handled without a visit. For this to work they both had established as a patient with me in person. As winter sets in many of us will suffer the effects of sunlight deprivation. In some people this can manifest as depression. I have personally found my energy levels drain during winter months and perk up in the Spring and Summer. As I have visited places further North I have found this effect to be particularly noticeable.
This brings me to the power of light. Not only is light important for getting adequate amounts of Vitamin D it also helps ameliorate depression in some people. An interesting study released in November in the Journal of the American Medical Association set out to examine the effects of light therapy on 122 adults suffering from moderate to severe depression. They had four treatment arms: Light monotherapy, Prozac plus light, Prozac monotherapy, and placebo. The results may surprise you. The best results came from combination Prozac and light therapy with light monotherapy coming a close second. Prozac monotherapy and placebo were equivalent. In conclusion, if you find yourself depressed consider light therapy especially if you have limited daily sunlight exposure.
